General

Medi-Mere adheres to the GDPR law, for the protection of privacy and personal data. Under this law, an organization that works with personal data has certain duties, and the individual whose data it is has certain rights. Personal data are data that provide information about your identity such as your name, (email) address, phone number, age, place of birth, and social security number (BSN). Medi-Mere processes personal data from:

  • Patients
  • Healthcare providers (referrers, specialists, midwives, etc.)
  • Employees of suppliers, scientific research researchers
  • Applicants

 

Duties of the general practice

General practices Medi-Mere is responsible for the processing of personal data that takes place in the practice according to the GDPR. The practice meets its obligations as follows: Your data is collected for specific purposes:

  • for healthcare provision;
  • for efficient management and policy;
  • for the support of scientific research, education, and information;
  • for recruitment and selection.
  • Your personal data is securely protected against unauthorized access.
  • Your personal data is not kept longer than necessary for good healthcare provision.

 

Processing of personal data by Medi Mere

Medi Mere only processes personal data if there is a legal basis for doing so. These bases are:

  • Consent given by you (Article 6(1)(a) GDPR)
  • Taking pre-contractual measures at your request and/or executing the agreement concluded with you (Article 6(1)(b) GDPR)
  • Compliance with legal obligations (Article 6(1)(c) GDPR)
  • Protecting vital interests of yourself or others (Article 6(1)(d) GDPR)
  • Performing a task carried out in the public interest or in the exercise of official authority vested in the controller (Article 6(1)(e) GDPR)
  • The pursuit of the legitimate interests of Medi Mere or a third party (Article 6(1)(f) GDPR)

 

Your rights as a data subject:

You have the following rights:

  • The right to know if and which personal data of yours is being processed.
  • The right to access and obtain a copy of that data (insofar as the privacy of others is not thereby impaired).
  • The right to correction, supplementation, or deletion of data if necessary.
  • The right to request (partial) destruction of your medical records.
  • This can only be complied with if retaining the data is not of considerable importance to another and the data does not have to be retained by law.
  • The right to add a personal statement (of a medical nature) to your file.
  • The right to object to the processing of your data in certain cases.

If you wish to exercise your rights, you can do this verbally or by using a request form made known to General Practices Medi-Mere. Your interests can also be represented by a representative (such as a written proxy, or your curator or mentor).

This privacy statement may be changed. Changes will be announced on the Medi Mere website. We recommend that you consult this privacy statement regularly.

 

Cookie Statement

Cookies

The General Practices Medi-Mere uses a session cookie and session storage on this website. Session cookies are functional cookies intended to make the website work properly. It is not necessary to ask the internet user for consent to use session cookies. These cookies contain a different, temporary number each time you visit the website. The session cookies are deleted after closing your browser.

 

JavaScript

We use a session cookie to check if your device, such as your computer, laptop, or smartphone, supports JavaScript. JavaScript ensures, among other things, that you see drop-down menus on the website. If your device does not support JavaScript, the website will not be displayed correctly.

 

Session storage

Session storage is a form of temporary data storage, similar to session cookies. We use session storage only to store information about the website’s structure in your browser. Website Statistics

We use the Google Analytics statistics program to analyze which pages are visited the most, how visitors come to our website, and what search terms are used in our search engine.

 

Duration of Storage

For this purpose, like every website, we collect the IP addresses of our visitors. These are stored in so-called log files. The log files are kept on the web server for 5 days to make them available. After that, the log files are kept for 90 days exclusively for security reasons and are only viewed for that purpose.
General Practices Medi-Mere has taken measures to limit the traceability of visitors to our website as much as possible.
We collect the following data with the log files:

  • cookies;
  • IP address;
  • User agents (browsers, operating system);
  • Search terms used to find our website via external search engines;
  • Search terms used in the search engine on the website itself;
  • Links used within the website;
  • Links used to arrive at our website.

These data are extracted by Google Analytics from the webserver’s log files. These log files remain in the database for 31 days. After that, they are deleted. Only an aggregated (combined) log file remains. This provides us with an annual report on website visits.
